Inside concrete slabs can settle due to a number of reasons, but one is often poorly compacted substrate. Builders do what they can to prepare the surface for the concrete but, sometimes there are hidden issues that they are not aware of, like buried organic matter (trees and stumps) that can degrade over time and cause voids.
Water issues can also cause concrete to sink. Either too much water, which can wash out the base under the slab, or even drought, which can cause soils to shrink, leaving voids underneath.
